Requesting a Leave of Absence

Overview

Asking for a leave of absence usually means a long form full of legal terms (FMLA, state paid family leave, short-term disability), filled in at one of the most stressful moments of someone’s life. Most people don’t know which leave applies to them, so they guess, call HR, or send a request that comes back incomplete.

I designed an AI-guided intake that replaces the form with a short conversation. It asks plain-language questions one at a time, works out which leaves the employee is likely eligible for, and shows them a visual plan of their time off, pay and job protection before they submit.
